All of the information I have give is true and complete. This cat will reside in my home as a pet. I will provide it with quality cat food, plenty of fresh water, indoor shelter, affection, annual physical examinations, and vaccinations under the supervision of a licensed veterinarian and will be spayed/neutered by 6 months of age.

If I cannot keep my adopted pet or afford medical treatment at any point in their life, I understand that I am required to return the pet to the Crazy Cat Lady Animal Rescue without asking for any reimbursement for care and will not take the adopted pet to any type of shelter, humane society, rescue group and will not re-home the adopted pet without first contacting us.

* The average life expectancy of a cat is 15 years and they can live 20+ years. Often people don't consider the cost of traveling with a pet or the challenges of finding a new home that allows pets. Please be mindful of the needs of you pet. Fosters are always needed.
